Right to Access
You have the right to inspect and obtain a copy of your health information maintained by SkinnyShots. To request access, submit a written request to our Privacy Officer at hello@skinnyshots.store.

You have the right to request that we amend your health information if you believe it is inaccurate or incomplete. Submit amendment requests in writing to our Privacy Officer.
Right to an Accounting of Disclosures
You have the right to request a list of instances in which we disclosed your health information for purposes other than treatment, payment, healthcare operations, and certain other activities.

You have the right to request that we restrict the use and disclosure of your health information. We are not required to agree to your request, except in limited circumstances.
Right to Receive Confidential Communications
You have the right to request that we communicate with you about your health information in a certain way or at a certain location. For example, you may request that we contact you only by mail or at a specific phone number.

You have the right to obtain a paper copy of this notice from us at any time, even if you have agreed to receive the notice electronically.
Our Responsibilities
